My business policy from Hill & Usher includes a provision for copyright infringement etc. but at relatively low limits ($5K I think).

As Nat mentioned I would look into the group policies you may be able to get through NANPA, ASMP, EP, etc. Companies like Hill & Usher will tailor what the group primarily needs and what also you need. There is plenty of options out there for insurance and it depends on what you shoot and also your market.

Just to be clear...this isn't insurance for copyright holder, it is insurance for people who download photos from the net or otherwise use a photo that turns out to be copywritten (accidently, of course).

Be careful everyone! Protect your intellectual property as best you can with insurance, IPTC data, watermarks, © logos, registrations, and even making your jpgs not the best quality. The internet is unfortunately a public domain medium. Someone may not have the ability to save an image, but they can simply do a PrtScrn and they got it.
